To register for the conference:
Please complete the form below. If registering for an agency, please put the agency name in the name fields and the contact person's email. Please then include all attendees names in the comment box.
To pay for the conference online:
Hit the purchase button under Pay for the Conference. This will bring you to the checkout page where you can select the amount of attendees that you are paying for.
To pay by check:
Please make the check out to MHBFSA. The check can be mailed with the completed registration form to: Selina Engle-Gross, 1226 N. State Road, Owosso, MI 48867
